stimulation (DBS) enables us to activate or deactivate regions of the brain by using electrical impulses in order to treat certain diseases or symptoms. DBS belongs to the functional neurosurgery that influences functions of the brain but is also reversible at the same time. DBS has evolved into an effective and recognized therapy of motion disorders with 80.000 treated patients worldwide. Deep brain stimulation is regarded as one of the most decisive developments in the treatment of neurological diseases over the last decades.<\/p><\/div><\/div><\/div><\/div><\/div><\/div><div class=\"vc_row wpb_row vc_row-fluid\"><div class=\"wpb_column vc_column_container vc_col-sm-12\"><div class=\"vc_column-inner\"><div class=\"wpb_wrapper\"><div class=\"wpb_text_column wpb_content_element \"><div class=\"wpb_wrapper\"><h2>What is treated with deep brain stimulation (DBS)?<\/h2><p>Motion disorders are treated with deep brain stimulation if they are caused by\u00a0<a title=\"Parkinson\u00b4s disease\" href=\"https:\/\/www.betaklinik.de\/en\/specialties\/neurosurgery\/parikinsons-disease\/\">Parkinson&#8217;s disease<\/a>,\u00a0<a title=\"Essential tremor\" href=\"https:\/\/www.betaklinik.de\/en\/specialties\/neurosurgery\/essential-tremor\/\">essential tremor (ET)<\/a>\u00a0or\u00a0<a title=\"Dystonia\" href=\"https:\/\/www.betaklinik.de\/en\/specialties\/neurosurgery\/dystonia\/\">dystonia<\/a>, if they could not be sufficiently treated with other therapies, or if the disease is very progressive. Severe psychiatric diseases have also been treated successfuly with DBS.<\/p><p>The contemporary surgical aiming points of DBS are:<\/p><ul><li>subthalamic nucleus (part of subthalamus) \u2013 in case of\u00a0Parkinson&#8217;s disease<\/li><li>globus pallidus internus (GPi) \u2013 in case of\u00a0dystonia<\/li><li>ventral intermediate nucleus (part of thalamus) \u2013 in case of\u00a0essential tremor (ET)<\/li><\/ul><\/div><\/div><\/div><\/div><\/div><\/div><div class=\"vc_row wpb_row vc_row-fluid\"><div class=\"wpb_column vc_column_container vc_col-sm-12\"><div class=\"vc_column-inner\"><div class=\"wpb_wrapper\"><div class=\"wpb_text_column wpb_content_element \"><div class=\"wpb_wrapper\"><h2>How does deep brain stimulation (DBS) work?<\/h2><p>During deep brain stimulation surgery, electrical electrodes are implanted in brain regions which have specific functions within the regulatory circuit of body movement. These brain areas are stimulated by using high-frequency, pulsatile, monopolar or bipolar electrical impulses. As a result, it is possible to selectively switch brain areas on and off. Symptoms can, thus, be switched on or off or at least reduced. The most important feature of DBS is that the function of the brain areas is sustained and the effect of the stimulation is reversible at any time. Despite of its clinical efficiency, the detailed mechanism of action in terms of motion disorder treatment with deep brain stimulation remains unknown.<\/p><\/div><\/div><\/div><\/div><\/div><\/div><div class=\"vc_row wpb_row vc_row-fluid\"><div class=\"wpb_column vc_column_container vc_col-sm-12\"><div class=\"vc_column-inner\"><div class=\"wpb_wrapper\"><div class=\"wpb_text_column wpb_content_element \"><div class=\"wpb_wrapper\"><h2>The advantages of deep brain stimulation<\/h2><p>The main advantage of deep brain stimulation in comparison to other procedures which destroy or remove brain tissue (pallidotomy, thalamotomy) is to adjust the stimulation subject to its effect. DBS is reversible without destroying or removing a huge amount of brain tissue. DBS does also not entail the partially grave side effects of drugs used to treat\u00a0Parkinson&#8217;s disease,\u00a0essential tremor (ET)\u00a0or\u00a0dystonia. In opposite to many neurological diseases forcing the neurosurgeon to operate a patient as quickly as possible due to the progress of the disease, deep brain stimulation surgery can be planned well in advance.<\/p><\/div><\/div><\/div><\/div><\/div><\/div><div class=\"vc_row wpb_row vc_row-fluid\"><div class=\"wpb_column vc_column_container vc_col-sm-12\"><div class=\"vc_column-inner\"><div class=\"wpb_wrapper\"><div class=\"wpb_text_column wpb_content_element \"><div class=\"wpb_wrapper\"><h2>Deep brain stimulation surgery \/ DBS surgery<\/h2><\/div><\/div><\/div><\/div><\/div><\/div><div class=\"vc_row wpb_row vc_row-fluid\"><div class=\"wpb_column vc_column_container vc_col-sm-12\"><div class=\"vc_column-inner\"><div class=\"wpb_wrapper\"><div class=\"wpb_text_column wpb_content_element \"><div class=\"wpb_wrapper\"><h3>Few days before surgery<\/h3><p>Few days before surgery, the head and the brain are examined using\u00a0<a title=\"MRI\" href=\"https:\/\/www.betaklinik.de\/en\/specialties\/neuroradiology-and-pediatric-neuroradiology\/mri\/\">MRI<\/a>. The images enable the surgeon to exactly plan the access and aiming points as well as the 3D access. The surgery can be simulated to minimize its risks. This examination is accompanied by further examinations and talks about the surgery, how long patients must have an empty stomach before surgery, about which drugs can still be taken, etc.<\/p><\/div><\/div><\/div><\/div><\/div><\/div><div class=\"vc_row wpb_row vc_row-fluid\"><div class=\"wpb_column vc_column_container vc_col-sm-12\"><div class=\"vc_column-inner\"><div class=\"wpb_wrapper\"><div class=\"wpb_text_column wpb_content_element \"><div class=\"wpb_wrapper\"><h3>Surgery day<\/h3><p>To implant the electrodes, a so-called stereotactic surgery has to be carried out. This is a minimally invasive surgery during which the head is fixed in a stereotactic frame in\u00a0<a title=\"Anesthesia and pain therapy\" href=\"https:\/\/www.betaklinik.de\/en\/specialties\/anesthesia\/\">local anesthesia<\/a>. The stereotactic frame serves to hold the head in an exact position.<\/p><\/div><\/div><\/div><\/div><\/div><\/div><div class=\"vc_row wpb_row vc_row-fluid\"><div class=\"wpb_column vc_column_container vc_col-sm-12\"><div class=\"vc_column-inner\"><div class=\"wpb_wrapper\"><div class=\"wpb_text_column wpb_content_element \"><div class=\"wpb_wrapper\"><p>A\u00a0<a title=\"CT\" href=\"https:\/\/www.betaklinik.de\/en\/specialties\/neuroradiology-and-pediatric-neuroradiology\/ct\/\">CT<\/a>\u00a0examination of the head makes it possible to create a patient-specific coordinate system as a reference for the integration of the\u00a0MR\u00a0images. A nearly distortion-free coordinate system is developed this way. The head of the patient is anesthetized with<a title=\"Anesthesia\" href=\"https:\/\/www.betaklinik.de\/en\/specialties\/anesthesia\/\">\u00a0local anesthetics<\/a>.<\/p><p>The skull is then opened by drilling a small hole through which combined stimulation and lead electrodes are guided 10 mm before the aiming point. By performing\u00a0<a title=\"Electrophysiology neurology and epileptology\" href=\"https:\/\/www.betaklinik.de\/en\/specialties\/beta-international-epilepsy-centre\/electrophysiology\/\">electrophysiological examinations<\/a>, the aiming points are identified by amplifying and measuring nerve cell activity (e.g., subthalamic nucleus). Neurological examinations can take place during the whole procedure because patients are awake.
